New Coaching Client Forms

Please complete all of the following steps prior to the first scheduled coaching session.

  1. Make Payment — choose one of the following options:
    • 6 Month Platinum Package - $1495
    • 3 Month Gold Package - $755
    • 1 Session Follow-Up - $175

    Payment may also be made by Personal Check or Money Order. Due to paypal processing expenses, a 5% discount is available for all payments made by Money Order or Personal Check in US funds:

    • 6 Month Platinum Package - $1420.25
    • 3 Month Gold Package - $717.25
    • 1 Session Follow-Up - $166.25
  2. Read the New Coaching Client Welcome and Agreement
  3. Read the Coaching Contract
  4. Complete the Client Intake Form, including the digital signature fields.
